AGREEMENT AND RELEASE OF LIABILITY

Voluntary Participation

I am freely and voluntarily choosing to attend and participate in this Club X event and to view and participate in activities that I know are adult oriented and sexually explicit and involve acts of domination, submission, bondage, discipline, sadism, masochism, and other explicit and extreme sexual and non-sexual acts including, but not limited to, spanking, paddling whipping, waxing, piercing, cutting, fire play, knife play, edge play, blood play, bondage, suspension, fisting, and oral, vaginal and anal contact and penetration. I understand that I may come in contact with the bodily fluids of others, either intentionally or unintentionally or accidentally, and I understand the risks associated with such contact. I understand that this contact and these activities involve significant risks including the risk of serious illness and injury and that these risks arise from the nature of the activities themselves as well as from the possible negligent, reckless or intentional conduct of the participants and attendees.

Assumption of Risk

I AM AWARE THAT THESE ACTIVITIES ARE CONSIDERED EXTREMELY HAZARD-OUS. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH FULL KNOWLEDGE OF THE DANGERS INVOLVED, AND I ACCEPT AND I ASSUME FULL RESPONSIBILITY FOR ANY AND ALL RISKS OF PROPERTY DAMAGE, PERSONAL INJURY AND ILLNESS, AND DEATH.

Release of Liability

As consideration for being permitted to enter this event, and to view and participate in these ac-tivities, and to use the facilities and equipment being provided, I agree that neither I nor anyone through me or on my behalf will make any claim against, sue, or attach the property of Club X, or anyone affiliated with Club X, including, but not limited to, its officers, directors, members, agents, employees, volunteers, monitors, promoters, sponsors and contractors, and including, but not limited to, the owners, lessors or suppliers of the premises, and the owners, lessors or suppli-ers of any equipment used in connection with the event, for any injury, damage or loss suffered by me at any time during my presence on the premises or at the event or as a result of my pre-sence at or participation in any activities at the event. I hereby release, waive and forever dis-charge them, their heirs, administrators, executors, affiliates, and assigns, from any and all ac-tions, claims, obligations, and demands that I, my assignees, heirs, distributees, guardians, and legal representatives now have or may hereafter have for any injury, illness, or damage, arising from or in any way related to my attendance at or participation in this event or these activities.

Indemnity and Hold Harmless

I agree to indemnify and hold harmless Club X and those individuals and entities described above from any loss, liability, injury, damage or cost that may occur, or that they may incur, as a result of my presence at or my participation in the event or arising from my actions at or in con-nection with the event, whether caused by my negligence, recklessness, willfulness, or otherwise.

Knowing and Voluntary Execution

I HAVE CAREFULLY RE AD THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND AN ASSUMPTION OF RISK AND A CONTRACT OF INDEMNITY BETWEEN ME AND CLUB X AND THAT I AM GIVING UP MANY LEGAL RIGHTS AND REMEDIES BY SIGNING IT.
